Graywhale CD Exchange (248 S 1300 East, SLC and several other locations around Utah): With 8 locations and all the used CDs you could ever hope for, Graywhale is one of the most popular local used CD stores. They do have new CDs, but the primary concern is used. Give yourself a lot of time to just browse, and don't miss the bargain racks. You never know what you might find.
The Heavy Metal Shop (63 Exchange Pl, SLC -- just west of State Street): As much hardcore punk as heavy metal, this place rocks hard and is one of the last true local and totally independent music shops around.
Mechanized (511 West 200 South #140 SLC, 801-521-5979): Technophiles, this is your record store. DJ equipment, vinyl, and you get bonus free supercoolness just for walking in.
Modified Music (1310 East 200 South, SLC, 801-355-1770): The best new (and used) cd shop for industrial, gothic, and 80s music. Modified also specializes in finding stuff you thought you'd never find, and they utilize a special nation-wide (maybe even international) search network that pretty much guarantees that they'll find it if it's available. Support them. They deserve your money more than those national chains.
Wasatch CD Exchange (539-1700; 65 West 100 South between Main and West Temple. Hours: Mon-Thurs Noon-10pm / Fri noon-11pm / Sun 11am-11pm): A new independent record store in Salt Lake City, with a great downtown location near shopping, restaurants and nightclubs. Wasatch CD Exchange carries a wide selection of music including rock, jazz, blues, world, reggae, classical, dance, rap, hip hop, country, bluegrass, rockabilly, etc. What really sets the store apart is its back room which has its own large sections for punk, metal, goth and industrial. It is one of the few stores in Salt Lake that buys, sells and trades both CDs and records. Wasatch CD Exchange also participates in Salt Lake City's new "Token" project. Spend $20 or more and receive a token good for an hour of metered parking, an hour in participating parking lots or a ride on TRAX or the bus.New info!

Clothes / Shoes
Accentuate! (208 E 500 S, SLC, UT): Goth-oriented gift shop and book exchange. Also includes some non-goth items (incl. unique and custom pet wear), vintage & gently used consignment, custom kanji, and a web store.
All For Love (3072 S Main St.): A pretty cool but tiny and cramped fetish shop featuring an "adult's only" room, lots of lingerie, fetish clothing and boots, etc. They have great sales, when you catch one happening. I doubt anyone else in the nation has ever had Catherine Coatney for 60% off the over-inflated prices that designer charges!
Arsenic Fashions: Beautiful, affordable corsets and original clothing and jewelry designs.
Blue Boutique (1080 E 2100 S): Just your usual hip little fetish shop -- plenty of vinyl clothes, an "adult's only" room, piercing, funky colored wigs, way over-priced gothy and punky fashions, etc. They're really expanded recently, and added a nice shoe / boot room.
Hot Topic (Southtown Mall, Crossroads Mall, University Mall, and soon to be in Fashion Place Mall): As all the MALL locations indicate, this may not be the most underground place around, but the undeniably hip and cool staff does go out of its way to accommodate local interest in clothing, gifts, and music for many subcultures (Goth, Raver, Fetish, and Punk). If you can't shop at locally-owned places, or you can't find what you're looking for anywhere else, this is the next best thing.
